BGS sandbox games have never been about the main questline for me.
The primary draw has always been roleplaying in the sense of creating my own storyline via exploration & discovery. Something I've yet to see other genre peers replicate at such a level.
In most sandbox rpgs, environments are just static set dressings for visual variety as I beeline from one objective marker to the next. With BGS the interactive environments are the primary role-playing vehicle.
Everything else is just gravy.

I looked at it again because I could be wrong, but this seems to point to NG+ required.New Game+ is just an addition, not a requirement to experience the new content (And the 2nd DLC indeed had general QoL improvements!).
